I'm a cigar smoker but I take the mini ones to Vegas. I play bartop VP and typically smoke if no one is around or if I'm around other smokers.

I smoke in Vegas. I always try to be courteous to those around me, including dealers - but I'm not going to refrain from smoking when I'm at a "smoking" table.
On this last trip, I was at the Gold Coast, playing head-to-head against the dealer at a smoking table.
A group of three came and sat down. They saw my ashtray & one girl says, "You're not smoking are you?".
I responded, "Well, not right now, but probably when my cocktail gets here."
She says, "If you're going to smoke, we're not going to play here."
Me: "OK....there are plenty of other tables. (I then light one up)... Good luck."

I would smile and respond: "No worries, I was leaving anyway." And then I would move. But that's a once-a-year occurrence.

People have conflicting standards of behavior. Smokers don't typically perceive any obligation to ask permission. Their viewpoint is: "Why are you in a casino?"

You're right about the 'asking permission' part.
But, an example of how I approach things is that I will skip any table where there are others playing and none are smokers. If there is only one other player, I will then ask if they would prefer to play alone rather than have me there smoking. If they don't care, then I sit down. If they do care, then I move on.
But once I'm in my seat & gambling at a smoking table? Nope. No permission will be asked for.

My take is that you are more courteous than most. I don't smoke. I would prefer that others at the table don't smoke. BUT.... I get it. The casino is not a smoke free environment. I have never seen a non-smoking craps table. If someone asks me, I would say I'm ok if they smoke, but I would prefer if they would not blow the smoke in my direction or hold their cigarette so that the smoke blows to me.

Anytime there are players (or dealer) at my table who are bothered by smoking, I put my ashtray in the chair next to me - typically to my left in the true 3rd-base seat. If I'm actually sitting at 'true-third', I tend to cup the cigarette in my hand & exhale straight down, toward the pit.
When pit-critters see this, they tend to pick up on it, and will constantly keep my ashtray emptied. This helps to avoid that awful dead-cigarette odor.
When there are other smokers at the table, I will try to wait on lighting up while others are already smoking. That helps keep the table from becoming a cloud-of-smoke environment - which is as annoying to me as it is to non-smokers.
Of course, I might be a bit different about it, as I am a non-smoker except for when I'm in Vegas.

On this last trip, I actually had a "smoking" room at the Fremont, since I told them I didn't care.
I won't be doing that again, as the 'stale-cigarette' odor really bothered me...way more than I thought it would.

I think I can endorse all the comments from topcard.

I avoid smokers if possible, and certainly if you've established yourself at a machine or table, I have no problem with you. My conflicts tend to involve hazards that make themselves unavoidable. An example would be cashing a ticket at a redemption machine, and turning to discover a smoker behind me with a cigarette four inches from my retina.

In general, I'm hostile to smokers on walkways. In Vegas, non-gambling and/or minor hotel guests are forced through the casino. They can avoid the machines and tables, but they can't avoid the walkways.

Used to be a full-time smoker but quit about 5 years ago, I will still smoke when in Vegas and a few special occasions throughout the year. I try and be a considerate smoker. When at the bj tables I will normally stand behind my chair when smoking so the smoke will not be bothersome to others. But if I am sitting at 1st or 3rd base and am trapped I will try to blow into the pit.

I smoke and spend a lot of my gambling time at VP bars. I normally search out a machine at the end so that I can turn and blow the smoke away from those sitting next to me. I'll also try to "shield" them with my body so smoke doesn't blow directly into their faces. I know I have the right to smoke but that doesn't mean I have to be a jerk about it. A little bit of courtesy goes a long way.
